Support required for phase 2 of the Better Lives programme - mobilisation and delivery of the Better Lives tool. It's purpose is to;
• embed the end-to-end realisation process in day-to-day practice
• target the cohorts where early movement will generate momentum and confidence
• use Better Lives data to inform live commissioning and market-shaping activity

This phase is designed to translate the opportunities already identified into measurable improvements in outcomes, begin to realise in-year financial movement, and embed the capability and governance required for Lincolnshire to sustain this approach at scale.
